Who we are
Nurture Health Well is a health and wellness practice mailing from PO Box 506, Church Hill, Tennessee 37642. We have been in practice since 2018. For privacy questions, email info@nurturehealthwell.com or call (423) 357-7386. Please do not send full medical records through the website form if a phone call would be safer.
Information you give us
When you use the booking form, the contact form, or the newsletter, we may receive:
- Your name, email address, and phone number
- The service you are asking about, and a preferred day or time
- Whatever you choose to write in the message
- The fact that you asked to subscribe or to unsubscribe
We ask only for what we need to reply. Please leave out Social Security numbers, full insurance member IDs, and photos of identity documents unless we have specifically asked for them on a secure channel.
How we use it
We use inquiry details to answer you, to schedule or decline a visit, and to keep a record of what was requested so the next person at the desk is not starting from zero. Newsletter details are used only to send occasional notes about everyday health from the practice, and to honor an unsubscribe.
We do not sell names, emails, or health details. We do not rent our list. We do not use your message to train advertising profiles.
Newsletter
The footer form adds you to a short mailing list. You can leave at any time on the unsubscribe page. Unsubscribing stops routine notes. It does not cancel an appointment request you already sent, and it does not stop us from replying about that request.
Who else may see it
Staff at Nurture Health Well who need the message in order to reply may see it. If we use a mail host or a phone service to receive the message, that provider handles it only to deliver the service to us. We share information with a clinician, insurer, or laboratory only when you have asked us to, when care requires it, or when the law says we must.
A form on a website is not the same as a sealed chart. Email can be copied or misdirected. If something is sensitive, call (423) 357-7386 and we will arrange a better way to talk.
How long we keep it
Booking and contact messages are kept long enough to complete the request and for the ordinary record of the practice, then deleted or filed with your chart if you become a patient. Newsletter addresses are kept until you unsubscribe, and a short record of the unsubscribe itself so we do not add you again by mistake.
Children
This website is written for adults arranging care. We do not knowingly collect information from children under 13 through the forms. A parent or guardian may write to us about a child. If you believe a child sent a form, email info@nurturehealthwell.com and we will delete it.
Your choices
You may ask us to:
- Tell you what contact details we hold from a website form
- Correct a name, email, or phone number
- Delete a newsletter address
- Limit how we use a message that is not already part of a medical record we must keep
Write to info@nurturehealthwell.com with the address you used. We may need to confirm it is you before we change anything. Medical records, once care has started, follow the retention rules that apply to the practice, which can be longer than a marketing list.
This website
Pages may remember a small amount of technical information so the site can load and so a form can show you that it was received. We do not run third-party advertising trackers. Links to maps, mail, or the phone dialer leave our pages and follow those services’ own policies.
Changes
If we change this policy in a way that matters, we will update the date at the top of this page. The copy you are reading governs the website forms. It does not replace a notice you may receive once you are a patient of the practice.
